Dear survey participants,

The diagnosis of a chronic illness or an accident can plunge us into a health crisis, leaving us and our loved ones unsure of how to cope with everyday life or how to continue with school or work.

 

In these situations, it is important to have someone to talk to who can explain what help is available. In hospital, the social services do this. Hospital stays are often short, though. Therefore, not all issues can be fully clarified before discharge.

 

With our project 'Moving forward together! (MFT)’, we want to incorporate patients and their families into a network of support services as soon as possible after discharge from hospital.

 

With this survey, we want to find out if our network of support services is working well for patients and their families. By taking part in our survey, you will be supporting the scientific monitoring of the project and helping to make a lasting difference in the lives of many patients and their families.

 

The survey will take no more than 15 minutes to complete. The survey is completely anonymous, and your answers will be treated confidentially.

This online survey is being conducted by Professors Michael Obermaier and Patrick Isele, both members of the Institute for Research and Transfer in Childhood and Family (foki) at the Catholic University of Applied Sciences NRW (katho), as part of the research project "Gemeinsam weitergehen! (GWG)" funded by Aktion Mensch and in cooperation with DiCV Paderborn and serves exclusively public welfare interests.   Consent: You agree that the information provided in the online questionnaire may be collected, stored, transmitted and - if you provide your e-mail address - also used for the purpose of reminding you to complete a new survey exclusively within the context of this project. The research data will be analyzed using scientific methods and used for publications resulting from it, as long as no conclusions can be drawn about your identity and person. Unless you revoke your consent beforehand, the data relating to your person or containing references to you will be deleted or made unrecognizable upon completion of the project. The research results and research data will be stored and archived in a research data center as proof of good scientific practice after the end of the project. Your consent is voluntary and you will not suffer any disadvantages if you refuse or revoke it.
